Link to a Third-Party Form on Your Portal

You can add links to third-party/external forms to your CivicPlus Portal using the Settings tab. 

Important Note

• CivicEngage Central and CivicOptimize forms for the organization will automatically sync to your Portal. Learn more about how to manage integrated forms. 

Instructions

1. Navigate to the page with your embedded Portal or access the stand-alone Portal
2. Select Settings > Forms
   
3. Select + New Form Link
   
   
4. Add the URL for the form
   
5. Add a Title and Description for the form
   
6. Click Save
   
   
7. Your form will now display on the Forms tab
