We recommend that every form contain the following sections and elements. View the Form Element Types article for more information.

Introduction

  • H1 element:

    • Text: Name of Form

    • Name: Name_of_Form

    • Size: Large

  • H1 element:

    • Text: Name of City (use ‘City of CivicCity’ for Form Templates)

    • Name: Name_of_Form (City_of_CivicCity)

    • Size: Medium

  • Information element:

    • Text: Name of City (use ‘City of CivicCity’ for Form Templates)

Form Content Elements

  • All relevant elements are based on process requirements

  • Forms should be presented on multiple pages if:

    • There are clearly defined sections of the form

    • There are conditional sections of the form

    • As a general rule, a single page should not contain more than 10 form elements

  • Avoid the use of conditionals for Form Templates
