View Advanced Fillable Forms Submissions

You can view Advanced Fillable Forms submission in both Recreation Management and Process Automation and Digital Services. This article will cover how to utilize each solution for viewing submissions.

View Advanced Fillable Forms Submissions in Recreation Management

View Fillable Form Submissions in Reports

  1. Log in to Recreation Management

  2. Navigate to the Reports tab

  3. Select and open a report from the following Report Categories:

    • User/Roster Reports

    • Reservation Reports

    Note

    Most common reports to use when viewing Fillable Form Submissions are Roster Reports and Facility Reservation Reports

  4. In the Filter tab, select Fillable Forms on the left navigation menu. Check the box for the Fillable Form(s) you want to see on this report

    Note

    Filtering by Fillable Forms will filter your report results to only sessions or facilities that have the selected Fillable Form(s) assigned to it within the session or facility configuration.

  5. In the Fields tab, select Fillable Forms on the left navigation menu. Check the box for the Fillable Form(s) you want to see on this report.

    Note

    Each selected Fillable Form will add an additional column to the report where you can see the submission status by participant or reservation holder.

  6. Click the Apply button

  7. Any participants or reservations with a completed Fillable Form associated will have a Download Form Submission button in the Form column. Clicking on this button will download the Fillable Form submission

    Note

    If an error occurs when clicking Download Form Submission button that states a Form is no longer available, this indicates the Form has reached its retention period configured in Process Automation and Digital Services.

View Fillable Form Submissions in User Accounts

  1. Log in to Recreation Management

  2. On the Accounts tab Search and Select which User Account you want to view

  3. On the Account Actions Menu select Form and Document Center

  4. From here you can view and download completed Fillable Forms submissions

View Fillable Form Submissions in Public View

  1. Log in to Recreation Management Public View

  2. Select the Account button

  3. On the right-hand menu, select Form and Document Center

  4. From here, public users can view, download and complete Fillable Forms as needed

View/Resend Fillable Forms in Activity List Session Roster

  1. Log in to Recreation Management

  2. Navigate to the Activities tab and then select Activity List

  3. Locate your Session and click on the Roster column View button

  4. In the Form Status column, click Completed next to the applicable participant to view/download the Fillable Form submission

  5. The Fillable Form Status Pop up with give you the option to download the completed form.

  6. If the Form Status says “Missing”, this indicates the participant has not yet submitted their Fillable Form. Click Missing to email the Fillable Form to the registrant or complete the form on their behalf.

    Note

    The email will be sent to the person who registered the participant. Public users can also complete these Fillable Forms from their Account Dashboard
