You can create custom field sets for your forms in Form Center. Field Sets are groups of fields that you can add to a form at one time.
Create a Field Set
Who can use this feature?
System Administrator | Owner | Publisher
Navigate to Modules and on the Content tab, select Form Center
Select the Field Sets tab
Select Add Field Set
Create a Name for your field set
Drag and drop Field Types and Field Sets into the empty space
Field Types
Short Answer: Will allow for a short answer (Character limit of 999 characters)
Long Answer: Will allow for a long answer (Character limit of 9,999 characters)
Checkboxes: Will allow you to add unlimited checkbox options to choose from
Dropdown: Will allow you to add unlimited drop-down options to choose from
Radio Buttons: Will allow you to add unlimited radio button options to choose from
Section Break: Will add a section break to the form
Reply Email: Will allow a reply email to be entered
Page Break: Will add a page break to separate the form
Date/Time: Will allow a date/time on the form
File Upload: Will allow files to be uploaded
Image: Will allow an image to be added
Link: Will allow the addition of a link
Text: Will allow for a text section
Masked Input: Will allow for masked input on an item
External Integration: Will allow you to add an external URL and instructions
Field Sets
Drag and drop existing Field Sets to your field set
If you add a Field Type, you will need to Label and Save Changes
Note:
Other required configuration options will vary based on Field Type.
Scroll to the top and click Save or Save and Make Active
Save: Will save the changes without making them active in the Form Center
Save and Make Active: Will save and make active to the Form Center
Return to the Main tab
Select a category
Find the form you want to add field sets to and click Actions
Select Modify
Locate the editor and drag and drop your Field Set into the box
Select Save

Edit the name of the field set
Drag-and-drop Field Types and Field Sets into the empty space

Field Sets
Drag and drop existing Field Sets to your field set
Click Save or Save and Make Active
Save: Will save the changes without making them active in the Form Center
Save and Make Active (Inactive only): Will save and make them active in the Form Center
Delete a Field Set
You must make a field set inactive before you can delete it.
Who can use this feature?

Navigate to Modules and on the Content tab, select Form Center
Select the Field Sets tab
Click Actions next to the desired field set
Select Delete
Respond OK to the pop-up to confirm the deletion
